Here is Dr. Kartush's Otologic Database - a great way to log your surgical cases. Windows and Mac versions are both available.

Both are stored in archives with READ ME files and databases included. Both require FileMaker Pro available from all the major mail-order firms for overnight delivery.

FileMaker Pro is one of the best and certainly the easiest database to learn. Simply changing a few field names will allow you to use use these "clones" for any type of clinical or surgical cases.
